Admission declaration

Are you being admitted to the hospital for a day or longer? Then you will need to sign an admission declaration on the day of your admission. The admission declaration provides an overview of your personal contributions to the costs.

Signing admission declaration

By signing the admission declaration, you confirm:

Your room choice
The rates and your personal contributions
The financial conditions of your admission

In case of emergency admission, you may sign the declaration as soon as you are able.
