Your relative has been admitted to a COVID-unit in UZ Leuven.

Since visits are no longer allowed due to COVID-19 measures, we would like to ask you to complete this online questionnaire to give us an idea of your relative's medical condition (physical and mental functioning, diet and social condition, etc.) prior to his/her admission to the hospital.
