Authorisation form to be signed by both partners before frozen material will be thawed.
Regarding the Belgian Law of July 6th 2007 concerning assisted reproductive medicine and the destination of supernumerary embryos and gametes, both partners need to sign an authorisation form before frozen material (embryos / sperm cells) will be thawed. It is only possible to start a cycle in which frozen material needs to be thawed, if the Leuven University Fertility Centre has received this completed authorisation form signed by both partners before the start of the thawing cycle.
Thawing cycles include both thawing cycles with the purpose of an embryo transfer, and thawing cycles where frozen sperm cells from the partner or from a donor will be thawed for a treatment cycle with the intention to perform intrauterine insemination (IUI) / in vitro fertilisation (IVF) / intracytoplasmic sperm injection (ICSI).
These authorisation forms will be included in each treatment package and will also be available in the waiting room.
